We designed the TELLTALE Electrosurgical Guidewire System for Transmural Systems, which won Silver at the 2022 Medical Design Excellence Awards, our second MDEA after Gold for Solea. HERNETICS has consulted to Transmural Systems since 2016.
TELLTALE supports the BASILICA procedure, which prevents coronary artery obstruction during TAVR. The critical step had been done by hand: the physician skives insulation off a guidewire with a scalpel, then pinches it to a kink angle by eye. Results varied with the operator dexterity, and a bad prep meant discarding the wire and starting over mid-procedure.
We designed a handheld denuder and kinker that captures the guidewire in a cavity and uses a rotating dial to skive a repeatable denuded area at mid-shaft, then a kinker tab to set the required angle. A jig, in the woodworking sense, it takes judgement out of the step that least benefits from it. The system also includes guide catheters, a spring-loaded connector cable, and guidewire grippers, designed for wet gloved handwork and cath lab body positioning.
